Key Responsibilities:
• Offer support and education in health and hygiene
• Advocate for individuals with complex health conditions
• Assist in implementing Individual Support Plans
• Provide nighttime and morning support to individuals
• Maintain communication among staff and external teams
Requirements:
• Certification in DSW, PSW, or equivalent
• Previous experience with high medical needs individuals
• Valid First Aid CPR/AED Level C certification
• Strong communication and interpersonal abilities
• Proficiency with computer systems for documentation
