As a Case Manager, you will develop and coordinate comprehensive support programs focused on enhancing self-esteem and mitigating isolation among youth. Collaborating closely with fellow staff members, you'll maintain consistent case management practices and ensure comprehensive youth support. The role includes daily travel to engage with youth at multiple sites.
Key Responsibilities:
• Create effective support programs that address youth needs
• Coordinate case management plans with the support team
• Maintain detailed electronic and written client records
• Foster community connections through youth engagement activities
• Utilize current technology trends relevant to youth communication
Requirements:
• Degree in Social Work or Child & Youth Care
• At least 2-3 years’ experience with at-risk youth
• Profound understanding of case management principles
• Completed Criminal Record Review Check and relevant training
• Access to a reliable vehicle and valid driver’s license
