Job Profile The Case Manager – Mental Health and Justice Program assists adults and youth (aged 12 years and older) who have the prospect of having a mental illness or dual diagnosis and are involved in the criminal justice system. The Case Manager collaborates with individuals to identify strengths and needs, develop long‑term goals, short‑term objectives, and concrete action steps to achieve those objectives. Individuals are assisted in acquiring skills and connecting with formal and informal support, peer support, qualified services, and resources to reduce the likelihood of further conflict with the law, enable progress toward recovery, and improve quality of life. Referrals to longer‑term services are made where required. The Case Manager also assists individuals and their supports in navigating the court system and offers support through information, referral, and consultation, working directly with court staff to explore possible outcomes and address the correlation between offenses and mental health needs. Responsibilities and Requirements Degree or diploma in health or social services. Degree plus four years or more experience in a community-based mental‑health setting; experience with the criminal justice system is an asset. Diploma plus six years or more experience in a community-based mental‑health setting; experience in mental health diversion is an asset. Current registration with the OCSWSSW or a regulatory body under the Regulated Health Professions Act. Knowledge of systemic issues and social determinants of health such as poverty, unemployment, stigma,
and the isolation felt by individuals with serious mental illness and their families. Knowledge of the Ontario Mental Health Act and mental health reform principles, the Substitute Decisions Act, the Health Care Consent Act, and PHIPA requirements. Ability to apply Recovery principles and empowerment‑oriented philosophies and practices in work with consumers. Second language reflecting the local community is an asset. Valid Canadian driver’s license, minimum $1,000,000 third‑party liability insurance, and proof that insurance covers the use of a personal vehicle to transport clients. Working Conditions Office work, community settings, meetings in clients’ homes and travel with clients.
