St. Joseph’s Home Care in Hamilton seeks a part time Social Worker for the Integrated Comprehensive Care Program to deliver psychosocial assessments, counselling, and case management to clients transitioning from hospital to home.
The SW-ICCP will support high-risk cases, coordinate services across community agencies, and contribute to mentoring and quality improvement. Regular daytime hours with travel within the service area are required.
