This permanent full-time role, operating Monday to Friday from 9:00 AM to 5:00 PM, is integral to helping individuals with reliable mental illness reintegrate after incarceration. You'll coordinate eligibility assessments, create supportive relationships, and assist clients in developing personalized recovery plans. Your work will connect them to essential resources like shelter, food, and health care.
Key Responsibilities:
• Review referral information of applicants to the program
• Conduct eligibility screening interviews with applicants
• Develop therapeutic relationships with assigned clients
• Complete comprehensive assessments of client needs
• Assist clients in creating personalized safety and recovery plans
Requirements:
• Experience in mental health or social services
• Strong communication and relationship-building skills
