AgeCare is looking for an experienced LPN to enhance our compassionate care model. You will report directly to the Director of Care, ensuring the delivery of patient-centered nursing services. Your expertise will contribute to developing accurate care plans and performing patient assessments, thus improving quality of life for our residents.

Key Responsibilities:
• Provide patient-oriented, goal-directed nursing care
• Complete and update clinical care assessments and documentation
• Implement and evaluate care/service plans and adjust as necessary




• Assist in scheduling and participating in resident care conferences
• Administer medications and direct resident care according to standards

Requirements:
• Practical Nurse Diploma with current registration
• Minimum 3 years of experience in seniors’ health
• Current CPR certification and proficiency in Microsoft Office
• Demonstrated leadership and mentorship skills
• Passion for providing high-quality, compassionate care
