Join Health Sciences North as a full-time Social Worker to support patients facing mental health and addiction challenges. Facilitate care plans and discharge processes while advocating for patient well-being.

This temporary role, lasting from August 2026 to September 2027, requires a dedicated social worker with at least one year of experience in healthcare, preferably in Mental Health and Addiction services. You will perform psychosocial assessments, develop care plans, and collaborate with families for successful discharges. Your expertise will help streamline access to community resources for our patients.

Key Responsibilities:
• Conduct psychosocial assessments to identify barriers
• Develop care plans with patients and caregivers




• Provide timely interventions for high-risk referrals
• Coordinate discharge plans with healthcare teams
• Facilitate family and case conferences as needed

Requirements:
• Bachelor’s Degree in Social Work required
• Valid registration with OCSWSSW needed
• Health and safety training certification required
• Minimum one year of relevant healthcare experience
• Solid communication and interpersonal skills required
