Upper Grand Family Health Team (UGFHT) in Fergus, Ontario seeks a Registered Kinesiologist to join the rural primary health care team. The role supports chronic disease management, prevention, and activity programs across Fergus, Elora and Arthur communities.
The successful candidate will work with an interdisciplinary team to deliver health promotion, falls prevention and cardiac rehab programs, including home visits and virtual care when appropriate.
