New Age Services is seeking a compassionate female Community Disability Practitioner for 20 hours per week. Ideal candidates will implement service plans, respond to personal support needs, and promote independence and inclusion for individuals with developmental disabilities, complex behaviors, and medically fragile conditions.
Key Responsibilities:
• Support daily living activities and recreation goals
• Implement and monitor individualized service plans
• Utilize trauma-informed practices for support
• Maintain accurate documentation as per agency standards
• Collaborate with families and healthcare professionals
Requirements:
• Valid Alberta Class 5 Driver's License
• Reliable vehicle and insurance of $2,000,000
• Comfort in aquatic activities with life jacket
• Willingness to work around pets
• Diploma or Degree in a related field
