Become a Relief Registered Nurse with Community South Fraser in Surrey, B.C. This role is dedicated to delivering essential health services and supporting chronic illness management.
In this full-time Nursing position, you will work independently and as part of an interprofessional team to manage client caseloads for adults and seniors with complex health needs. The role emphasizes assessments, direct care, and the creation of actionable health plans. Key Responsibilities:
• Develop therapeutic relationships with clients and families
• Conduct comprehensive assessments and care planning
• Advocate for client needs and health transitions




• Maintain explicit and accurate client records
• Participate in quality improvement initiatives
Requirements:
• Current registration with BCCNM as a Registered Nurse
• One year of experience in community or outpatient care
• BC Driver’s license and personal transportation
• Strong problem-solving and communication skills
• Knowledgeable about community health resources
