Key Responsibilities:
• Review and assess referrals for eligibility
• Collaborate with community partners on discharge planning
• Conduct intake interviews and rehabilitation assessments
• Create personalized rehabilitation service plans for clients
• Continuously monitor service delivery and provide support
Requirements:
• Master’s degree in Social Work is required
• Background in coordinating services for brain injuries
• Solid collaboration and communication skills
• Effective organizational and time management abilities
• Must be registered with BCCSW
