We are hiring a full‑time, permanent social worker or occupational therapist to serve a small case load (8‑10) of complex mental health clients, many with co‑occurring substance use disorders.
Responsibilities
- Provide direct case management, including assisting clients with physical and mental health appointments, housing, social/rec programming, financial management, and other services as needed.
- Coordinate care with a multidisciplinary team that includes a team lead, other ICMs, and support workers.
- Apply crisis‑management techniques, harm‑reduction principles, trauma‑informed care, and person‑centered philosophies.
- Collaborate with clients to develop and monitor individualized service plans.
Qualifications
- Valid driver’s license and ownership of a reliable vehicle.
- Current First Aid/CPR certification.
- Excellent teamwork and leadership skills.
- Solid crisis‑management skills.
- Knowledge of harm‑reduction, trauma‑informed, and person‑centered philosophies.
- Must be able to become a registered social worker or occupational therapist; registration cost will be covered by Phoenix.
Schedule
Monday‑Friday, four day shifts and one evening shift (1:00 pm‑9:30 pm).
