LMC Healthcare is seeking a passionate, dedicated Diabetes Educator - Registered Dietitian for a permanent full time position. You will educate patients in both one-on-one and group settings on self-management skills for diabetes and other conditions. Collaborating with a multidisciplinary team, you'll develop care plans and conduct thorough patient encounters while employing advanced information technology.
Key Responsibilities:
• Educate patients on diabetes self-management skills
• Develop care plans with multidisciplinary collaboration
• Complete EMR documentation for patient encounters
• Teach insulin administration and SMBG techniques
• Conduct follow-ups via various communication methods
Requirements:
• Valid RD license with the College of Dietitians of Ontario
• Experience in teaching individuals and groups
• Working towards CDE designation
• Valid driver's license and personal vehicle access
• Health coaching experience or willingness to train
