Here, you’ll conduct comprehensive client assessments, develop and implement personalized care plans, and lead collaborations with the care team. Key Responsibilities:
• Perform comprehensive client assessments and develop tailored care plans
• Act as a primary provider for a designated caseload of clients
• Collaborate with community support services for client transitions
Requirements:
• Current registration with the College of Nurses of Ontario
• Previous CNA specialty certification is an asset
• Valid driver’s license and reliable vehicle
• Supportive of community care and evidence-informed practice
• Ability to work overnight shifts from 11pm-7am
