Position Summary

This is a permanent part‑time role (not locum) to augment a family practice with a long‑term care (LTC) panel, caring for frail elderly patients.

Responsibilities

Point Grey Private Hospital is a 155‑bed LTC home currently supported by four LTCI practitioners; an additional practitioner is needed to assist with the resident caseload.

Practitioners will provide holistic, resident‑centered care in partnership with an interdisciplinary team (physicians, nurse practitioners, nurses, allied health, and care‑home leadership). Responsibilities include:

- Regular proactive visits to residents
- On‑site medication reviews
- Attendance at care conferences




- Participation in 24/7 on‑call and after‑hours care program
- Complete documentation in charts

Qualifications

Competent physicians experienced in family practice with an interest in long‑term care. Mandatory to meet five best‑practice expectations of LTCI (see duties above). No prior LTC experience required; a supportive community of practice is provided to build skills and confidence.
