Key Responsibilities:
• Coordinate client rehabilitation and liaise with the team
• Attend home visits and medical appointments
• Manage service referrals and transportation needs




• Complete paperwork for medical reimbursement
• Provide detailed reports for medical-legal requirements
Requirements:
• Master's Degree in Social Work, Occupational Therapy, or Physiotherapy
• Minimum 3-5 years clinical experience
• Valid Driver's License and access to a vehicle
• Registration with a regulated college
