Impact lives as a full time Physiotherapist within the Toronto Western Family Health Team. This role engages at Bathurst and Garrison Creek sites, enhancing rehabilitation and health outcomes for patients.

As an essential part of our interdisciplinary team, you will focus on individualized care through detailed assessments and customized treatment plans. Collaborating with healthcare professionals, you'll also engage in educational efforts and research initiatives aimed at improving physiotherapy services and patient outcomes.

Key Responsibilities:
• Assess and consult on patients' physical function.
• Develop and adjust treatment plans in partnership with patients.
• Re-assess and analyze treatment successes and setbacks.
• Educate staff and the community on physiotherapeutic practices.




• Take part in quality improvement and research projects.

Requirements:
• Master's Degree in Physiotherapy or recognized equivalent required.
• Active registration with the College of Physiotherapists of Ontario essential.
• Minimum one year of relevant clinical experience.
• Experienced in chronic condition assessment and treatment.
• Strong problem-solving and clinical judgment skills required.
