As a Nurse Practitioner in the Extended Class, you will manage patient care autonomously, assess and diagnose conditions, and coordinate treatment plans. Key Responsibilities:
• Assess, diagnose, and create comprehensive patient care plans
• Coordinate and manage patient care throughout treatment
• Serve as the primary contact for patients and families
• Facilitate timely transitions and follow-ups in care
• Consult with interprofessional team for optimal outcomes
Requirements:
• Registered Nurse in the Extended Class in Ontario
• Minimum 3 years of clinical nursing experience
• Master’s degree in Nursing or equivalent
• Evidence of strong clinical decision-making and leadership skills
• Ability to work autonomously and collaboratively
