Join the Family Navigation Hub at Holland Bloorview Kids Rehabilitation Hospital as an Ambulatory Care Registered Nurse. Provide innovative pediatric nursing care in this temporary full-time role.

As a Registered Nurse, you will focus on establishing a nursing role in the Family Navigation Hub team. Required expertise includes pediatric rehabilitation, family-centered care, and collaborative practices. You’ll support families and children with complex medical needs while promoting independence through education and care planning.

Key Responsibilities:
• Provide evidence-based nursing care to clients
• Develop treatment plans with clients and families
• Monitor client progress and implement care recommendations




• Coordinate referrals and community resource support
• Collaborate with interdisciplinary teams and community providers

Requirements:
• Current RN registration with the College of Nurses of Ontario
• Minimum one year pediatric nursing experience preferred
• Certification in Pediatric Nursing is a plus
• Knowledge of rehabilitation settings and community resources
