Based at Toronto General Hospital, you'll collaborate closely with the UHN CaRE Clinic team. Your responsibilities include monitoring Medly alerts, educating patients and families on self-care, and overseeing guideline-directed medical therapy. This role demands a proactive approach to patient management, ensuring documentation is meticulously maintained within the EPIC system.

Key Responsibilities:
• Recruit pilot participants and obtain informed consent
• Manage daily Medly alerts in collaboration with the clinic team




• Communicate with patients on proper Medly usage
• Provide education on adverse reactions and self-care
• Support guideline-directed medical therapy and medication titration

Requirements:
• Bachelor of Science in Nursing; Master’s preferred
• Registered Nurse with 3-5 years of experience
• Current registration with Ontario College of Nurses
• Clinical research experience preferred
• Strong organizational and problem-solving skills
