You'll coordinate eligibility assessments, create supportive relationships, and assist clients in developing personalized recovery plans. Your work will connect them to essential resources like shelter, food, and health care. Key Responsibilities:

- Review referral information of applicants to the program
- Conduct eligibility screening interviews with applicants
