Serve as a Clinical Dietitian for Sunnybrook’s Academic Family Health Team. Focus on providing nutrition support and education to enhance patient care and community outreach. This part-time position requires a dedicated Registered Dietitian to assess and address patients' unique nutritional needs. In collaboration with a vibrant interprofessional team, you will help to implement care plans aligning with patient backgrounds and lifestyles.

Your role will include quality improvement initiatives and actioning community health education sessions. Key Responsibilities:

- Analyze patient nutrition through comprehensive assessments
- Execute customized medical nutrition therapy based on assessments
- Create detailed reports within EMR (PSS)




- Work closely with interdisciplinary teams on care plans
- Develop educational outreach sessions for the community Requirements:
- Bachelor’s degree in Human Nutrition or equivalent
- Good standing registration with the College of Dietitians of Ontario
- Diabetes Educator Certificate (preferred)
- Minimum 2 years of hands-on patient experience
