In this role, you will oversee discharge planning for CalAIM-eligible individuals while ensuring compliance with nursing standards and regulatory guidelines.

You’ll collaborate closely with healthcare providers, monitor utilization data, and provide clinical oversight for reentry care plans. Your expertise will facilitate safe and effective transitions of care for vulnerable patients in a mission-driven organization. Key Responsibilities:

- Coordinate clinical reviews and utilization management activities
- Review documentation and pre-certifications for compliance




- Collaborate with care teams to support discharge planning
- Monitor claims and referral activities for accuracy
- Provide oversight of reentry care plans and transitions Requirements:
- Current RN license and BSN preferred
- Two years of clinical experience in discharge coordination
- Previous experience in correctional facilities
- Maintain all RN licensure and certifications
