Job Details

Part Time: 2 shifts per week, 1:3 weekends plus an On‑call rotation. Minimum two shifts per week plus 1:3 weekends and statutory holiday rotation.

Pay Rate: $34.76 - $37.64 per visit.

Primary Functions

- Provide comprehensive nursing care to palliative patients in their homes within the Halton area.
- Organize the day, set priorities based on continuity of care, the type of care to be given and the client’s health needs.
- Develop, implement, evaluate, and modify the plan of care outlining the strategies, orders and actions to achieve expected/desired outcomes.
- Assess the health status of the client.
- Administer IV therapy including central line and peripheral line maintenance.
- Administer medications topically through natural body orifices, through tubes and by injections.
- Handle wound care, acute and chronic, for certain types and stages of wounds.
- Ensure that the laptop is synchronized with the network at a minimum of twice a day, at the beginning and end of shift, to receive information on and update client records.
- Attend team/POD meetings as required.





Position Requirements

- Minimum 1 to 2 years clinical practice in a community setting preferred.
- Minimum 1 to 2 years of nursing experience preferably in palliative care, hospice, or home health.
- Current registration with the CNO.
- Certification in Palliative or hospice care (preferred).
- Current CPR certificate.
- Excellent interpersonal, communication and teaching skills.
- Valid driver’s license and access to a vehicle.

Must be legally eligible to work in Canada.
