As part of the Harvest House Atlantic Housing Team, the Housing Case Worker will support tenants in permanent housing units to develop tenancy skills and achieve life goals through action-oriented case management.
To ensure that HHA continues in its leadership role in community for quality systems and data management, case management activities and delivery on goods and service outcomes will be tracked in the appropriate modules of Hifis
Duties and Responsibilities
- · Complete a move in assessment tool to identify housing needs, food security needs, continuity of care plan for any health and mental health & addiction service needs and support tenant to facilitate any action items identified.
- · Through provision of client-led support, guide the tenant to establish a successful tenancy through meeting of immediate needs, creating community connection, and assisting with community navigation, as required.
- · Respond to tenant crisis, as required, through strengths-based, trauma- informed approaches
- · Use appropriate assessment tools, case management resources, and readiness rulers to assess and support a client-led action-oriented case plan to achieve client life goals.
- · Engage with community partners to better serve tenants.
- · Maintain all Hifis 4 files in appropriate modules.
- · Attend mandatory professional development and recertification programs as scheduled.

- 100% of crisis incidents documented in HIFIS within 24 hours
- 90% of crises responded to within same day
- Reduction in repeat crisis incidents by 20% annually
- 85% of tenants report feeling supported during crises (via surveys/feedback)
4. Community Engagement & Resource Navigation
- Minimum 2 community referrals per tenant per quarter (as needed)
- Establish/maintain at least 5 active community partnerships annually
- 80% of tenants engaged in at least one external support service
- Documented follow-up on 100% of referrals within 14 days
5. Documentation & Data Management (HIFIS 4)
- 100% of client interactions documented within 48 hours
- 95% data accuracy rate in file audits
- 100% compliance with required HIFIS modules
- Zero missed reporting deadlines

- 100% attendance at required trainings and certifications

Pay: $22.00-$24.00 per hour
