INTAKE / CHARGE NURSE

About the Position In collaboration with the Nurse Manager, Medical Director, and the broader Hospice leadership team, the Intake/Charge Nurse (RN) plays a key role in coordinating and overseeing daily clinical operations. This position serves as the primary point person for the Hospice triage and admission process and provides direct supervision and support to clinical staff. As a frontline leader, the Charge Nurse is vital to maintaining excellence in care delivery and ensuring adherence to best practices and regulatory standards.
Duties and Responsibilities:
Hospice Triage and Admissions
- Coordinate screening of patients for appropriateness of admission following standardized triage protocol.
- Triage referrals, manage the Referral List, and respond to referral sources as per NS hospice standards.
- Maintain timely communication with other providers (hospital staff, family doctors, palliative care teams) during triage and prior to admissions.
- Oversee and coordinate admissions, meeting patients and families upon arrival to complete the admission process.
- Conduct community tours and respond to inquiries from prospective patients and family members.
Supervision and Support of the Clinical Care Team
- Provide clinical leadership and oversight during assigned shifts, serving as a resource for problem-solving and coordination of care.
- Coach, motivate, train, and provide ongoing feedback to clinical staff (RNs, LPNs, and CTAs) in palliative care delivery.
- Oversee scheduling and planning for safe patient care, including managing contingency plans for staff absences.
- Address conflict in the workplace with the support of management and hold team members accountable for standards of care.
Quality Assurance and Care Delivery
- Conduct routine audits of patient records to verify that nursing documentation meets legal and professional standards.
- Ensure accurate and safe distribution, tracking, and disposal of all medications and supplies.
- Conduct daily rounds with the attending physician to ensure quality care and provide guidance as required.
- Assume all responsibilities of the Registered Nurse role when necessary.
Qualifications and Experience
- Education: Current registration with the Nova Scotia College of Nursing (NSCN) as an RN.
- Experience:
Minimum 2 years of clinical nursing experience in hospice palliative care.
- CNA Certification in hospice palliative care, or working towards it.
- LEAP Certification
- Current CPR Certification.
- Leadership: Experience in a formal management or previous charge nurse role is an asset.
- Skills: Demonstrated leadership, mentoring, and problem-solving skills with a compassionate approach to work.
- Technical: Proficiency with computers (Microsoft Office/Google Workspace); experience with electronic medical records (eMAR/eCharts) is an asset.

Compensation and Benefits
- Base Salary: Starting salary $84,344 Annually (based on $40.55/hour at 2080 hours)
- Position Premiums: In addition to the base salary, this role receives a Charge Premium of $2.00/hr.
- Additional Premiums: $3.50/hr weekend premium (Saturday and Sunday) and $75.00 per 24-hour period for On-Call duties.
