Position Overview The social worker is part of an interdisciplinary team and is accountable to the clinical manager of the Geriatric Psychiatric Service.

This professional is expected to provide services aligning with the “Behavior Supports Ontario” initiatives in the community.

He/she provides liaison services to facilitate clients’ return to their home following hospitalization.

The social worker will provide intervention and support services to the clients and their families and/or caregivers.

The social worker also has a consultative and education/teaching role in the community such as day programs, staff in senior residences, natural caregivers, as well as the community at large.

The social worker is expected to be mobile and visit clientele in their settings.



This professional must have the ability to organize and manage their schedules and appointments in order to respond to demand in due time, as well as the autonomy and clinical judgment to assess and prioritize received requests from various sources.

HOURS OF WORK

Full-time Regular – 1.0 FTE

SALARY

Salary of BSW as per O.P.S.E.U. Collective Agreement

What we are looking for

- Bachelor degree in Social Work




- Member in valuable standing with College or Professional Association in Ontario.
- 3 years of recent experience with the geriatric population.
- Must be fluently bilingual in verbal and writing
- In-depth knowledge of mental health issues and psychiatric diagnoses, as well as best practices in these areas
- Experience in case management and system navigation is an asset.
- Experience in psychoeducation, teaching or coaching colleagues is an asset
- Ability to document clinical interventions clearly, following HGH approved documentation policies and procedures.
- Valid driver’s license and access to own vehicle.
- Willingness to travel and work in various rural settings/environments: home settings, retirement homes, acute care hospitals.
- In-depth knowledge of dementias and other mental health/cognitive disorders in seniors.
- In-depth knowledge of behavior issues related to dementia and other cognitive disorders.
- Knowledge of geriatric assessment and best practices.
- Knowledge of the Gentle Persuasive Approach (GPA).
- Enhanced knowledge of family and caregiver needs.
- Competency in delivering support/assistance through education and behavior modification.
- Demonstrated knowledge and best practices in dementia and other cognitive losses, delirium, and their impact on individuals, their families and caregivers.
- Good problem solving ability as well a clinical judgment, good interpersonal and communication skills
- Good capacity to work in an interdisciplinary team.
