As a Support Coordinator, you will provide person-centered planning while addressing practical needs such as finances and housing. This role serves individuals with severe and persistent mental illnesses, facilitating connections to community supports and resources. Your contributions will aid in enhancing clients' lives through effective crisis intervention and versatile support planning.
Key Responsibilities:
• Facilitate person-centered recovery planning for clients
• Develop and implement support plans for various life aspects




• Coordinate community resources and advocate for clients
• Evaluate and modify planning effectiveness regularly
• Create effective crisis plans with people supported
Requirements:
• Undergraduate degree/diploma in psychology or social work
• Minimum three years of community mental health experience
• Strong crisis intervention and communication skills
• Knowledge of Recovery philosophy and community resources
• Valid driver’s license with own transportation
