Vancouver Coastal Health (VCH) is a leading health system that implements minimum Nurse to Patient Ratios (mNPRs) in partnership with the Ministry of Health and the BC Nurses’ Union to enhance quality of care.
Salary range: CAD $49.20/Hr. – CAD $63.47/Hr.
Older Adult MHSU Outreach Nurse (RN/RPN)
This role works primarily with older adults who have dementia or major psychiatric disorders, concurrent disorders, or other conditions associated with aging. The nurse provides psychogeriatric assessments, crisis intervention, case coordination, treatment planning, and consults with general practitioners, caregivers, and family members. The role also facilitates individual and group sessions, offers counseling and education, and establishes partnerships with continuing, residential, and hospital care teams. Additionally, the nurse may provide direct care and outreach services as needed.
Responsibilities
- Conduct psychogeriatric assessments and create individualized treatment plans.
- Deliver crisis intervention and coordinate emergency response.
- Facilitate and co‑facilitate individual and group counseling sessions.
- Consult with clinical teams and community resources to ensure continuity of care.
- Provide educational programs and supports for clients, families, and staff.
- Act as a clinical resource for nurses and other health providers.
- Maintain documentation in accordance with VCH policies and BCCNM standards.
- Adapt to changing shift patterns and operational requirements.
Qualifications
- Current registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives.
- Three years of recent experience in geriatric mental health and addictions or an equivalent combination of education and experience.
- A valid B.C. driver’s licence and reliable vehicle access.
Knowledge, Skills & Abilities
- Deep knowledge of gerontology, geriatric psychiatry, and Canadian ageing care standards.
- Proficiency in assessment and treatment of psychiatric conditions in older adults, including dementia, delirium, depression, and psychotic disorders.
- Understanding of concurrent disorders, substance use management, and harm‑reduction models.
- Experience with psychopharmacology, crisis intervention, and community resource navigation.
- Strong counseling, psycho‑education, and consultation skills with families and multidisciplinary teams.
- Ability to act as liaison between hospital and community services.
- Familiarity with family dynamics, end‑of‑life issues, and advance care planning.
- Ability to conduct assessments with validated tools and formulate holistic care plans.
- Excellent communication skills, both oral and written, with clients, families, and health professionals.
- Capacity to work independently and collaboratively, lead, and facilitate group activities.
- Skill in CPR, nursing equipment usage, and relevant computer applications.
- Physical capability to perform the duties of the position.
Work hours, including days off, may change to meet operational needs and are governed by the Collective Agreement and applicable statutes.
