Salary range: CAD $44.85 - $56.02 / hour
Job Summary
Provide social work services to improve the emotional and social consequences of a health condition for clients and families. Promote effective coping and adaptation to illness, mastery of situations, maintenance of social roles, safe and timely discharge to home or other levels of care.
Responsibilities
1. Identify and engage eligible clients using social work high‑risk screening and case‑finding techniques.
2. Conduct social and psychosocial assessments of patients and families and interpret the information using professional standards and clinical practice.
3. Provide therapeutic counseling services to clients and families experiencing a variety of circumstances related to their illness.
4. Facilitate a process whereby the patient and family identify their needs for recovery, adaptation and mastery of the situation; promote client/family autonomy through participation in planning and decision‑making and develop problem‑solving abilities.
5. Follow up and provide ongoing monitoring of psychosocial situations for clients, evaluate responses to interventions, and revise the care plan as necessary.
6. Provide information regarding client psychosocial situation as a member of the interdisciplinary team; participate in developing a comprehensive plan of care with/for the client; collaborate and consult with other community professionals.
7. Act as a resource for clients and families by providing information about community resources and facilitating referrals where appropriate and supporting the development of community resources.
8. Help clients explore reactions to loss and provide counseling from a strengths perspective to sustain motivation and hope.
9. Coordinate and advocate for the client with other service providers, both internal and external to the organization, to promote maximum client social functioning.
10.
Participate in quality improvement activities by providing input into the development, implementation, and evaluation of program goals, objectives, policies and procedures.
11. Educate other health service providers about the psychosocial aspects of the health problem through participation in discharge planning rounds, patient and family conferences, clinical documentation, and consultations.
12. Document clinical interventions and participate in outcome assessments and workload measurement activities.
13. Perform other related duties as assigned.
Qualifications
Education and Experience
Bachelor's degree in Social Work from an approved school of Social Work plus two (2) years of recent related clinical social work experience working with individuals and families with mental health and substance use issues.
Current full registration with the BC College of Social Workers. Valid BC Driver's License and access to a personal vehicle for business‑related purposes.

- Demonstrated knowledge of counseling skills and clinical practice models as it relates to individuals, families, and groups.
- Ability to conduct social and psychosocial assessments.
- Ability to provide leadership to the interdisciplinary team in care planning for complex client situations.
- Ability to work effectively with others, including in a multicultural environment.
- Ability to work collaboratively as part of an interdisciplinary team and understanding the scope of practice of other health care disciplines.
- Ability to make effective referrals, reflecting in-depth knowledge of community resources and solid collaborative skills with community agencies.
- Demonstrated ability to communicate effectively both verbally and in writing with clients, families, other members of the health care team, and community agencies.
- Demonstrated ability to effectively deal with conflict.
- Ability to facilitate group meetings, processes, and decision making.
- Ability to operate related equipment.
- Physical ability to perform the duties of the position.
