About the role:

The Clinical Consultant provides clinical oversight, consultation, and coordination for the agency's intake processes, clinical practice standards, and quality improvement initiatives. The role supports the delivery of high-quality, person-centered services through clinical consultation, staff support, policy development, program evaluation, and interdisciplinary collaboration.

Duties and Responsibilities

- Coordinate the Agency's intake processes, including the review of referrals, eligibility screening, informed consent, and transition of clients into services.
- Respond to inquiries from clients, families, caregivers, referral sources, and community partners, providing information, clinical consultation, and support with system navigation.
- Provide clinical consultation and guidance on complex client situations, care planning, risk management, ethical decision-making, and evidence-informed practice.
- Coordinate the development, implementation, review, and evaluation of clinical policies, procedures, standards, quality improvement initiatives, and best practices.
- Review clinical documentation and provide consultation, coaching, and education to promote professional practice, documentation quality, and legislative compliance.
- Facilitate clinical audits, monitor referral and service performance indicators, analyze trends, and prepare reports to support organizational planning and continuous quality improvement.
- Support interdisciplinary collaboration by facilitating clinical meetings, case conferences,



and coordinated service planning.
- Serve as the agency's Ombudsperson and Privacy Officer, supporting the Agency's privacy, complaints, and clinical governance processes, ensuring compliance with PHIPA and other applicable legislation.
- Develop and maintain collaborative relationships with hospitals, funders, community agencies, and other stakeholders to strengthen service coordination and continuity of care.
- Participate in community education, awareness initiatives, and the Agency's on-call rotation as required.
- Advocate for equitable access to services and positive client outcomes while supporting the Agency's strategic priorities.
- Perform other duties related to the position.

Qualifications and Requirements

- Undergraduate degree in Social Work or a related health or human services discipline. A Master's degree is considered an asset.
- Current registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW), or eligibility for registration, is considered an asset.
- Minimum 4 years experience providing clinical consultation, case coordination, or rehabilitation programming is required.
- Experience working with adults living with acquired brain injury and knowledge of rehabilitation, healthcare, and community support systems is preferred.
- Demonstrated knowledge of the Personal Health Information Protection Act (PHIPA), privacy legislation, and confidentiality best practices.
- Experience working in a unionized environment is considered an asset.
- Strong organizational, time management, prioritization, and project coordination skills, with the ability to manage competing priorities.
- Demonstrated ability to work independently while fostering collaborative relationships within an interdisciplinary team environment.
- Excellent critical thinking, clinical reasoning,



problem-solving, conflict resolution, and crisis management skills.
- Strong interpersonal, verbal, written communication, report writing, presentation, and facilitation skills.
- Proficiency with Microsoft Office Suite and electronic clinical documentation systems is considered an asset.
- A valid Ontario Class G driver's licence, reliable access to a vehicle, and proof of business-use automobile insurance are required.
- Ability to perform the essential duties of the position, including travel throughout the community as required.

Hours of Work

- 40 hours per week, with flexibility required to meet operational, client, and program needs.

Working Conditions

- Primarily in-office, with travel to community locations and partner organizations as required.
- Regular use of computers and other office technology.
- Occasional evening or overtime work may be required to meet operational needs.
- Participation in the Agency's on-call rotation, as required.

Pay: $70,000.00-$81,000.00 per year
