Position Summary
Criminal Justice Ministry (CJM) is seeking a compassionate, skilled, and mission-driven Clinical Life Coach & Housing Stabilization Specialist to support individuals experiencing severe mental illness, substance use disorders, chronic homelessness, and justice involvement. This position plays a critical role in helping participants stabilize, maintain housing, build independence, and move toward long-term wellness and self-sufficiency.
The Clinical Life
Coach & Housing Stabilization Specialist will provide intensive, community-based support through a trauma-informed, person-centered approach and will work directly with clients in their homes and throughout the community.
Essential Responsibilities
- Provide intensive case management and life coaching services to individuals experiencing chronic homelessness, severe mental illness, substance use disorder, and justice involvement.
- Conduct regular in-home and community visits to assess participant stability, safety, and overall wellbeing.
- Support medication management and treatment adherence through coordination with healthcare and behavioral health providers.
- Utilize motivational interviewing, trauma-informed care, and person-centered practices to support engagement and behavior change.
- Facilitate connections to community supports and resources, including healthcare, mental health treatment, recovery supports, transportation, benefits, and peer services.
- Maintain timely and accurate documentation and participate in required reporting and team meetings.
Qualifications
- Master’s degree in Social Work (MSW), Counseling, Nursing, Psychology, Human Services, or a related field.
- Experience working with chronically homeless populations and/or justice-impacted individuals.
- Experience supporting individuals with serious mental illness and substance use disorders.
- Knowledge of trauma-informed care, motivational interviewing, harm reduction, and person-centered practices.
- Reliable transportation, valid driver’s license, and proof of insurance required.
