Key Responsibilities:
• Complete home evaluations within 24 hours of referral
• Develop individualized treatment strategies and track progress
• Educate clients and families on safety and equipment




• Document all assessments and communications clearly
• Coordinate care with an interdisciplinary team to optimize outcomes
Requirements:
• Experience in home/community care strongly preferred
• Registered with the College of Occupational Therapists of Ontario
• Proficiency in safety evaluations and ADL retraining
• Valid driver’s license with reliable transportation
• Ability to manage independent schedules and documentation
