This full time role includes Monday to Friday hours with on-call responsibilities for evenings and weekends. Your leadership will drive effective care coordination and employee performance monitoring, keeping policies aligned with best practices. You will play a vital role in inspiring our healthcare teams and enhancing public relations for Community Health Services.
Key Responsibilities:
• Coordinate and supervise Community Support Workers in the field
• Develop and maintain care plans for Community Health Services




• Monitor policies of the Regulated Health Professions Act
• Market Community Health Services positively to the public
• Provide guidance on workplace Health and Safety
Requirements:
• Registered Nurse (RN) or Registered Practical Nurse (RPN)
• 3-5 years of relevant experience or equivalent
• Registration with the College of Nurses of Ontario
• Computer proficiency in scheduling and documentation
• Current CPR Certification
