Position Summary

Intake Coordinator, Child Mental Health

You will provide direct operational support to patients and families experiencing acute psychiatric condition(s), by coordinating and triaging all internal and external referrals to the Child and Adolescent Mental Health Outpatient program. You have superior communication and interpersonal skills, effortlessly connecting with physicians, staff, patients, and families, and display a high degree of professionalism, with the ability to work independently, in small groups and in a team environment. You have the ability to make rapid and informed decisions with confidence in addition to being highly organized and able to work in a fast-paced environment.

This position supports the hospital’s academic mandate by advancing care through teaching and learning and/or transforming care through research and innovation.



This position contributes to the shared responsibility of ensuring health and safety policies are followed to provide for a healthy environment for patients, staff, physicians, volunteers and families.

On a practical level, you will

• Process incoming internal and external referrals.

- Triage referrals to most appropriate service or clinician.
- Act as a team resource for transitioning patients to outside programs.
- Liaise between sites for Child and Adolescent patient care and program issues.
- Develop a standard intake process with team input.
- Participate in quality improvement activities as required
- Participate in unit/program/Hospital committees and initiatives as required

Qualifications

• Post-secondary College Diploma required
- Bachelor’s Degree in a health-related field preferred
- Minimum one year of experience in a children and adolescent mental health program required
- Previous intake experience preferred.
- Experience using a computerized triage system required
- Strong computer skills, including MS Office Suite
- Bilingualism in French/English an asset
