As a key member of the team, the Nurse Practitioner will:Perform assessment, diagnosis and treatment, health education/counselling and other clinical activities according to the College of Nurses of Ontario standards of practice for Registered Nurses in the Extended Class (NPs)



as well as the Medical Directives of the weCHCParticipate in active outreach in the region/community to assist client's access to primary health care and serve as a connection to community physicians/inpatient services in hospitalsWork collaboratively with consulting physician and the health care team to provide optimal careProvide clinic assessments addressing the physical and emotional needs of clients with multiple co-morbid conditionsContribute to chart documentation as per college standards, participate in chart reviews and case conferencesMake referrals to support client needs and goalsInitiate the development, implementation, monitoring and evaluation of treatment, education, counselling and health promotion programs and services as appropriateImplement appropriate strategies by working with the inter-professional team using a client centered approachActively participate in the teaching/learning process with clients, families, students and team members about best practice and evidence-based carePerform other duties that support the mission and vision of the organization and/or as assigned based on position responsibilities and/or site requirements.SKILLS,



KNOWLEDGE & EXPERIENCE REQUIREDMinimum 1-2 years experience as an Extended Class Nurse Practitioner working with vulnerable clientsExperience working with mental health and addictions is an asset.Experience in a community health setting or combination of community health and hospital or public health settingsStrong attention to patient-centred care and establishing therapeutic relationships requiredDemonstrated ability to work in an inter-professional team environment requiredStrong communication and excellent interpersonal skills, combined with good analytical, problem solving, organizational and administrative skillsProficient in the use of computers and various software applicationsClean driver's abstract, current, valid Class "G" Driver's License and reliable transportation to support travel across Windsor-Essex County and the province, as requiredBilingual (English/Spanish) language skills a strong asset.EDUCATION, TRAINING & CERTIFICATE REQUIREMENTSUndergraduate and/or Graduate degree in Nursing from a recognized UniversityQualified to provide care to clients across their lifespanMember in good standing in the Extended Class with the College of Nurses of OntarioMember in good standing with the Nurse Practitioner's Association of Ontario (NPAO) and/or Registered Nurses Association of Ontario (RNAO)Training and/or certification related to mental health and addictions recommended considered an assetExperience with phlebotomyCompletion of a Police Clearance and Vulnerable Sector Check prior to commencing employment. #J-18808-Ljbffr
