Job Title: Nurse Practitioner (NP)

Location: Scarborough, ON (Community-Based)

Employment Type: Part-Time, Permanent (17.5 hours/week) or Full-Time, Permanent (37.5 hours/week)

Compensation: $52.83 – $74.83/hour

About the Role

Prosperity Workforce Solutions is hiring on behalf of a community health organization in Scarborough seeking a compassionate and skilled Nurse Practitioner (NP) to provide comprehensive, client-centered primary care.

This role focuses on health promotion, disease prevention, and the management of acute and chronic conditions, with a strong emphasis on supporting underserved and diverse populations. You will work within a collaborative, interdisciplinary team to deliver accessible, high-quality care to children and families in a community-based setting.

Key Responsibilities

- Deliver comprehensive, family-centered primary care within scope of practice
- Assess, diagnose, treat, and manage acute and chronic conditions
- Conduct client assessments and develop evidence-based care plans
- Provide routine and catch-up immunizations in alignment with public health guidelines
- Maintain accurate and confidential electronic medical records (EMR)
- Collaborate with an interprofessional team to optimize patient outcomes
- Participate in case conferences, quality improvement, and clinical audits
- Support health promotion initiatives and community outreach
- Provide care to marginalized populations, including those with mental health and substance use concerns
- Conduct home visits as required
- Participate in on-call rotation with physician support
- Support mentorship and education for students and team members

Qualifications

Education & Registration:

- Master's degree as a Nurse Practitioner (Primary Health Care)




- Current registration in good standing with the College of Nurses of Ontario (Extended Class)
- Membership with NPAO and professional liability coverage
- Current CPR (HCP level) certification

Experience

- 3–5 years of NP experience preferred; new and early-career NPs are encouraged to apply
- Experience in primary care settings (Community Health Centre, Family Health Team, NP-led clinic) strongly preferred
- Experience working with diverse and underserved populations is an asset
- Program development and community health experience considered an asset

- Excellent communication and interpersonal skills
- Proven ability to collaborate within an interdisciplinary team
- Strong organizational, planning, and research skills
- Familiarity with EMR systems
- Demonstrated cultural competency and sensitivity
- Additional language skills are considered an asset

Working Conditions

- Adaptable schedule, including occasional evenings and weekends
- Participation in an on-call rotation with physician support
- Community-based role including home visits
- Valid driver's license and access to a vehicle required

Compensation & Benefits
