Key Responsibilities:
Assess individualized patient needs using appropriate tools
Execute and monitor ongoing care plans
Build rapport with patients and engage collaboratively
Maintain skilled patient records accurately
Provide on-call coverage as needed for safety
Requirements:
CNO Registration as a Registered Nurse in good standing
Valid driver’s license and vehicle access required
Experience in community care is an asset
Flexibility and reliability in scheduling
Willingness to travel to patient residences
Support patient independence and comfort as a community-focused nurse with Para Med.
