As a member of the multi-disciplinary health care team, the Registered Practical Nurse will collaborate in the provision of therapeutic services for stroke clients.
days, evenings, nights, weekends, holidays The Integrated Stroke Model of care provides assessment and treatment to stroke patients along the continuum. The stroke system begins in acute care on the 24 bed unit on 7South at the General Site. Stroke recovery continues on B2North, a 28 bed unit in the Regional Rehabilitation Centre co-located at the HGH site.
The team also works closely with the Restorative Care Program at St. Peter's Hospital where longer stay stroke rehabilitation occurs for some patients. Staff will have experience with skills in the treatment of stoke patients in both the acute and rehabilitation phases of the care continuum.
This unique model provides improved access to best practice and rehabilitation care for all individuals who have suffered a stroke.
- Current Certificate of Registration from the College of Nurses of Ontario as a Registered Practical Nurse
- Holds current certificates in Medication Administration, Enteral Nutrition Via Gastrostomy Tube, Urinary Bladder Catheterization, Transcription of Physicians Orders, Advanced Wound Care and Airway Management, or has completed equivalent community college studies
- Demonstrated knowledge and 2 recent years therapeutic experience within the last 5 years with clients who have had a stroke
- Demonstrated knowledge and 2 recent years therapeutic experience within the last 5 years in the field of neurological rehabilitation with the stroke population
- Demonstrated interpersonal, communication and teamwork skills
- Demonstrated commitment to interdisciplinary care and the principles of professional practice
- Demonstrates judgement in carrying out his or her role or function and is responsible and accountable for acting on the interpretation of data
- Demonstrates critical thinking and decision making in assessment and problem identifications
- Demonstrates ongoing personal professional education and development
