Step into the role of Clinical Coordinator within the Epilepsy Team at UHN's Toronto Western Hospital, focusing on patient management and team-oriented care planning. This full-time, temporary position emphasizes both clinical expertise and patient interaction.
You will be instrumental in navigating patients through the entirety of their epilepsy care journey, from treatment planning to post-operative follow-up. Your role will integrate clinical coordination with education and quality improvement initiatives, ensuring that patient care standards are upheld in a supportive setting. Excellent communication and organization will be vital to effectively manage complex cases.
Key Responsibilities:
• Lead clinical coordination across the epilepsy care pathway
• Act as primary liaison for EMU patients and families
• Optimize admissions and triage complex patient issues
• Track patient outcomes and support educational initiatives
• Contribute to policy development in collaboration with teams
Requirements:
• Relevant nursing registration with Ontario authorities
• Bachelor's degree in Nursing needed
• At least 2 years of practical experience in healthcare
• Robust relationship-building and communication skills
• Computer proficient in Microsoft Office and EPIC systems
