Provision of Social Work services to clients and families in Odette's Patient & Family Support Program caring for patients with breast cancer at an advanced level of practice within an interprofessional model. This includes: completion of psychosocial assessments; leading or participating in Advance Care Planning and/or Goals of Care conversations; provision of counseling and emotional support; conducting group work; locating and arranging resources; consultation/collaboration; teaching; participating in research; administration and professional development.
Qualifications/Skills:
Masters Degree in Social Work required.
Must be a current member in good standing with the OCSWSSW.
Superior communication, negotiation, mediation and team-oriented interprofessional team skills required.
Well-established knowledge of relevant health care system processes and community resources required.
Demonstrated solid commitment to academic practice required.
Minimum 5 years' previous experience working in a health care setting preferred.
Demonstrated Psychosocial Oncology knowledge preferred.
Psychosocial provision of care to those with breast cancer preferred.
Proven excellent assessment, counseling and problem solving skills essential.
Superior time management, organization, and prioritization skills in addressing multiple tasks and timelines in a fast-paced setting required.
Proven commitment to person and family-centred care essential.
