Key Responsibilities:
• Develop comprehensive health care plans for patients
• Provide psychosocial support for chronic and acute illnesses
• Offer patient education and make service referrals
• Promote health and traditional practices in care
• Facilitate case management interventions
Requirements:
• Baccalaureate in a health-related Science
• Strong writing and communication skills
• Crisis management and intervention experience
• Registered or eligible for licensing in health
• Valid driver's license and vehicle for work
