Key Responsibilities:
• Facilitate communication between patients and healthcare professionals
• Assess patient discharge and transition needs accurately




• Educate families about coping strategies for lifestyle changes
• Develop integrated discharge plans collaboratively
• Support patient/family-centered care effectively

Requirements:
• Bachelor’s degree in a health-related discipline
• One year of experience in case management required
• Registration in good standing with a qualified college
• Knowledge of relevant healthcare legislation
• Proficiency in English; French considered an asset
