Job Description Opportunity to Take Over an Established FHO/FHT Group Practice in 2027

A familyphysician that is a member of a 2-member group practice within a 26-member FHO onthe Southwest Hamilton Mountain is looking for a candidate to take over herpractice in early 2027. This is an excellent opportunity for someone who islooking to locum long-term with the intention to take over the practice.

Thepractice has a diverse roster with patients of all ages and backgrounds andreceives administrative support from 5 clerical staff including 2 receptionists(shared between the group), book-keeping/billing staff,



and support staff forbooking appointments/tests. As part of the Hamilton Family Health Team, thepractice also receives extensive support from allied health professionals, including significant nursing support provided by 3 Registered Nurses (1full-time and 2 part-time) and 1 full-time Nurse Practitioner. Additionalallied health support from a clinical pharmacist, dietitian, psychiatrist, anda mental health counsellor is also provided.

Thepractice is currently uses P&P; EMR and in office facilities include 4 examrooms (3 for the practicing physicians and 1 shared with the NP), a privateoffice/charting area, and a reception area/waiting room. The location alsohosts a pharmacy, x-ray/ultrasound clinic, physiotherapy clinic, and dentaloffice, and parking is free for both staff and patients.

Convenientlylocated on the fast-growing Southwest Hamilton Mountain and just minutes awayfrom the Lincoln Alexander Parkway, this well-equipped practice is easilyaccessible.
