This role emphasizes working directly with Palliative and Oncology patients within a collaborative multidisciplinary team. You will provide comprehensive assessments, treatment coordination, and psychosocial support, including individual counseling and group work. Additionally, you will facilitate access to resources and advocate for clients, advancing social work services that promote health and well-being.
Key Responsibilities:
• Conduct comprehensive assessments for designated clients
• Develop and coordinate care plans including treatment services
• Provide direct psychosocial care and counseling
• Advocate for client resources and navigation assistance
• Facilitate education and self-management support for clients
Requirements:
• Registered Social Worker certification in British Columbia
• Valid driver's license for travel requirements
• Experience in Palliative or Oncology settings preferred
• Solid collaborative skills with multidisciplinary teams
