Key Responsibilities:
• Coordinate patient transfers between health care facilities

• Travel to service locations to ensure patient support

Requirements:
• Bachelor of Social Work or Nursing required
• Minimum of three years’ experience in health services
• Valid Manitoba Class 5 Driver’s License essential
• Robust communication and organizational skills
• Knowledge of Indigenous health issues necessary
