At AgeCare Brandt's Creek Mews, we are looking for an experienced LPN to deliver patient-oriented care while collaborating within an inter-professional team. You will utilize your 3+ years of seniors’ health experience to follow care plans and maintain accurate documentation. This is an chance to work closely with the Director of Care and contribute to various aspects of residents' care.

Key Responsibilities:
• Implement and evaluate effective care/service plans
• Conduct clinical assessments and ensure documentation is up to date
• Assist with resident care conferences and communicate changes




• Facilitate smooth transitions for residents with the Director of Care
• Administer direct resident care and medications

Requirements:
• Practical Nurse Diploma and current registration
• 3+ years of experience in seniors’ health
• Current CPR training and computer proficiency
• Leadership, coaching, and mentorship skills
