Join the One Door Chronic Disease Management Team as a Registered Dietitian, delivering expert nutrition education to patients, especially those with chronic and complex conditions. You will utilize clinical practice guidelines and promote health through disease prevention strategies. A valid driver's license and transportation are essential as services are provided throughout Pictou County.
Key Responsibilities:
• Provide nutrition care for diverse client conditions
• Deliver health education focused on disease management
• Collaborate within the Primary Health Care Program
• Follow clinical guidelines for patient dietary needs
• Travel to various Primary Health Care Centres
Requirements:
• Bachelor’s in Human Nutrition from recognized university
• Dietetic internship completion required
• Licensure by the Nova Scotia Regulator of Dietetics
• Certification with Canadian Board of Certified Diabetes Educators
• Minimum two years' related experience required
