Our client is seeking a Nurse Practitioner (NP) to join their team providing primary care and addiction services to individuals impacted by homelessness and substance use.
Key Responsibilities
- Provide primary care and addiction services using a client-centered approach
- Conduct assessments, diagnosis, and treatment for clients with complex physical and mental health needs
- Collaborate within an interprofessional team and make community referrals as needed
- Maintain accurate EMR documentation and support quality improvement initiatives
- Promote evidence-based practices and contribute to clinical protocols and policies
Qualifications
- Master’s Degree in Nursing (MSN) and Primary Health Care NP (NP-PHC) certification
- Current CNO registration (Extended Class)
- 1+ years’ NP experience in primary care preferred
- Experience supporting individuals facing homelessness or substance use
- Valid Ontario driver’s license and access to a vehicle
- CPR certification and vulnerable sector police check required
- Flexibility to work across multiple sites, including one evening per week

Pay: $130,000.00-$135,000.00 per year
