Registered Nurse (RN) / Registered Practical Nurse (RPN) – Urgent Hire | Complex Home Care

Right at Home is urgently seeking an experienced and compassionate Registered Nurse (RN) or Registered Practical Nurse (RPN) to provide one-on-one nursing care to a client in their home.

This position is ideal for a nurse who is confident working independently and has experience with tracheostomy care, G-tube feeding, complex medical needs, and palliative/end-of-life care.

Key Responsibilities

The RN/RPN will provide individualized nursing and supportive care based on the client's care plan, including:

- Tracheostomy care and monitoring
- Tracheostomy suctioning and airway management, as required
- G-tube feeding and medication administration through G-tube
- G-tube site care and monitoring
- Medication administration and medication monitoring
- Monitoring and documenting vital signs
- Monitoring respiratory status and changes in the client's condition
- Palliative and end-of-life care
- Comfort measures and symptom monitoring
- Skin integrity monitoring and pressure-injury prevention
- Repositioning, mobility and personal care assistance as required
- Infection prevention and control
- Light housekeeping related to the client's immediate care and environment
- Accurate nursing documentation and visit notes
- Promptly reporting changes in the client's condition
- Communicating with family members, the care team and other healthcare professionals as appropriate
- Providing compassionate emotional support to the client and family

Qualifications





- Current registration in good standing with the College of Nurses of Ontario (CNO) as an RN or RPN
- Previous experience with tracheostomy care is strongly preferred
- Experience with G-tube feeding and care
- Experience with palliative/end-of-life or complex home care is an asset
- Current CPR/First Aid certification
- Current Vulnerable Sector Check
- Strong assessment and clinical judgment skills
- Ability to recognize and appropriately respond to changes in a client's condition
- Comfortable working independently in a client's home
- Reliable, compassionate and professional
- Strong communication and documentation skills

Schedule

Urgent availability required.

Shifts may include 12–13 hour visits, depending on the client's needs.

Pay: $28.00-$40.00 per hour
